At Victor Nitu DDS, we specialize in a variety of stabilized full-arch replacement options, including both removable bar retained dentures and ball retained dentures. Additionally, we offer a non-removable denture option known as the All-on-4 system. This provides the patient, a complete fixed dental bridge that is supported by a series of dental implants. This immensely beneficial system has revolutionized the way our patients live with dentures, vastly increasing their function and feel.
What Is All-On-4?
The All On 4 treatment combines the patient’s denture with four dental implants. A dental implant is a titanium rod that is surgically implanted into the jaw bone. The two posterior implants are longer in length, designed to capture thicker bone, they are placed at a precise angle to maximize the strength and support of your jawbone. The two anterior implants are placed at a vertical angle like conventional implants where less chewing force is used.
The All-On-4 Procedure
As with any treatment, the All-on-4 placement process begins with a consultation. During your consultation, Victor Nitu, DDS will collect information and begin a series of diagnostic tests to create a treatment plan. Candidacy for treatment is contingent on the patient’s ability to heal properly. Patients who have decreased healing, may need to make some lifestyle changes, or changes with their physician prior to treatment. Our initial preparatory work may include digital x-rays, CBCT scans, and other 3D images.
Please inform our team of all medications you are taking, prescribed or not, whether they see relevant or not. We want to understand all aspects of our patient’s health. The All On 4 treatment generally does not require a bone graft procedure due to the depth of the dental implants used for this surgery, though we will want to help the patient have healthy gums to reduce the likelihood of infection developing following surgery. With a treatment plan in place, we can schedule your procedure.
On the day of surgery, we will have already created a treatment map, detailing precise placement and angles of the where the dental implants will be placed. We will consult with the patient on pain relief options, which often include one of several conscious sedation options that we have available.
Once comfortable, we will make four small incisions in the gums to expose the underlying bone and place the dental implants. With the bone exposed, we then use a specialized dental drill to create four tiny holes in the jaw. The dental implants are deeply embedded into the jaw. Though this may sound extreme, the entire process is only minimally invasive and safe for even our medically delicate patients.
With the dental implants in place, we can then attach the abutments, which are support structures to link the screws to your restoration. With the abutments, we can then install a prefabricated denture restoration. The All On 4 system is one of the few dental implant treatments that allow for same-day restoration placement. Most dental implant-supported restorations need to be placed at a later date.
It is important to note that the first prosthetic we place is temporary. Once the healing process is complete, we will take an impression of your mouth and fabricate a permanent fixed bridge. Most All On 4 bridges are made from porcelain or zirconia.
